Online web forms can be a liability if not used with caution on the Drupal platform. In order to mitigate this risk, requests will be considered based on the below answers. Please complete this form as comprehensively as you can.

Under no circumstances are web forms allowed to be used for the submission of any type of assignments for any type of NWU-registered students. The NWU online learning platform eFundi is the tool that should be used for this purpose.

The web form and all the data submitted will be removed one month after the form is closed and it is therefore the responsibility of the faculty/department/unit to backup data.
