I declare that I am acquainted with the provisions of the Promotion of Access to Information Act (PAIA) and that this statement serves as an application in terms of the said legislation. I declare that I am acquainted with the Protection of Personal Information Act (POPIA) and the definition of personal information as contained in the Act’s definitions section. I furthermore undertake to ensure that the information I obtain will be accessible only to officers and persons directly involved in the project/research and will be handled and managed on a strict “need to know” basis and to inform other stakeholders of the personal/confidential/sensitive nature of the information. I undertake that if I store the information in any electronic format/medium that I will take all the necessary security measures to protect the information provided to me. I further undertake to submit the final draft of my research to the supervisor/promoter/editorial committee in order to determine whether the conditions have been met, if the research is not completed for any reason whatsoever, I will destroy the personal/confidential/sensitive material and inform the University’s Registrar accordingly. I agree that if the promoter/supervisor/editorial committee is of the opinion that the agreement was not fully complied with, the matter will be recorded and debated by the Registrar and /or a duly authorised representative. If consensus is not reached during such discussion, the matter will be referred to an arbitrator outside the University. This arbitrator will be appointed by both parties involved. Both parties will be given the opportunity to explain their case/views, in a manner determined by the arbitrator. The finding of such arbitrator will be final.
